By booking services with RenicA Transitional Healthcare Solutions, LLC, you agree to the following terms: RenicA provides non-medical care support, including care coordination services, education, and consulting designed to assist individuals and families during transitions from hospital to home and throughout recovery. Our services are tailored to client needs and may include: Home safety assessments, care coordination guidance, non-clinical medication organization guidance, patient and caregiver education, and patient discharge assistance. All services are private pay and are not billed to insurance. Payment is required prior to or at the time of service unless otherwise agreed upon in writing. RenicA reserves the right to: refuse or discontinue services if safety concerns arise, modify services based on client needs and scope, and update policies at any time.
RenicA Transitional Healthcare Solutions is a non-medical consulting service offering care coordination services. While our services may be led or informed by a Registered Nurse, it’s important to note that RenicA does NOT provide:
- Medical diagnosis
- Medical treatment
- Skilled nursing care
- Medication administration
- Emergency medical services
All recommendations provided are educational and supportive in nature, designed to complement non-medical care support, and should not replace advice from:
- Your physician
- Licensed healthcare providers
- Emergency services
In the event of a medical emergency, clients are instructed to call 911 immediately.
Participation in RenicA services does not establish a:
- Patient-provider relationship
- Clinical care agreement
Additionally, we also offer patient discharge assistance to help ensure a smooth transition.
All services, including our care coordination services and non-medical care support, are fee-for-service (private pay). Accepted payment methods may include: Credit/Debit Card, Electronic Payment (Square, online payment link), and approved alternative methods. Payment expectations are as follows: Initial services, including the Transition Visit and patient discharge assistance, are due at booking or prior to visit. Packages must be paid in full or according to the agreed payment plan. Failure to submit payment may result in cancellation or delay of services. Returned or failed payments may incur additional fees.
We understand that life happens, especially when it comes to coordinating care. However, due to the nature of our scheduled in-home visits for care coordination services, the following policies apply:
- A 24-hour notice is required for cancellations or rescheduling.
- Cancellations made with less than 24 hours notice may incur a cancellation fee.
- Same-day cancellations may be charged up to 50–100% of the service fee.
- No-call/no-show appointments may be charged in full.
At RenicA, we will make reasonable efforts to reschedule services, such as non-medical care support and patient discharge assistance, when appropriate.
Due to the time, preparation, and scheduling involved in our care coordination services, we have the following refund policy:
- Completed services are non-refundable.
- Deposits and booking fees for non-medical care support are non-refundable.
- Prepaid packages may be eligible for a partial refund of unused services at RenicA’s discretion, particularly in cases involving patient discharge assistance.
Refund considerations may include:
- Extenuating circumstances
- Service delivery issues
- Client satisfaction concerns (handled on a case-by-case basis)
All refund requests must be submitted in writing.
RenicA values your privacy and is committed to protecting your personal information in relation to our care coordination services. Information collected may include: contact details, health-related background (for service planning), and caregiver/family involvement details. Your information will: be kept confidential, only be used for coordinating non-medical care support, and never be sold or shared without your consent (unless required by law). By using RenicA services, including our patient discharge assistance, you consent to the collection and use of information for care coordination purposes.
By engaging with RenicA, you agree to:
Receive communication via phone, email, or text regarding our care coordination services.
Allow coordination with family/caregivers when authorized for effective non-medical care support.
Participate actively in your care planning process, including patient discharge assistance.
You may opt out of communications at any time.
RenicA Transitional Healthcare Solutions shall not be held liable for: Outcomes related to medical conditions, decisions made by clients or families, and actions taken outside of provided recommendations. Clients acknowledge that they are responsible for their own healthcare decisions. While RenicA provides guidance—not medical directives—it is important to understand that our care coordination services, non-medical care support, and patient discharge assistance are designed to help you navigate your healthcare journey.
By booking services such as care coordination services, submitting forms, or making payment, you acknowledge that you have: Read and understood these Policies & Disclosures, agreed to all terms outlined above, and consented to participate in services, including non-medical care support and patient discharge assistance, under this framework.
